What did the infestations update even add and how does it work?

I've been traveling to a few zones that are supposed to have an infestation but nothing appeared. I've also only seen an infestation circle once. I traveled into it, stood around with a few people and didn't see any enemies and suddenly it was gone. What the hell is this?
