F21 with M49 Fiancé. Need serious advice, feel trapped in relationship and idk what to do?

F21 with M49 fiancé. No income, no license due to seizures, no support system. He controls everything and I feel trapped. How do I get out safely? I’m 21F and I’ve been with my 49M fiancé we’ll call him “Matt” for over a year and I feel trapped, helpless, and like I’ve lost control of my own life. I have nowhere left to turn so I’m hoping for advice.

*My situation right now*

No income, I work as a private PCA, but I don’t get hours anymore. The agency I work for resigned with the Presbyterian home in my area, so my schedule dried up. I have no other job and no money of my own. No license, I have ongoing unidentified seizures. The state won’t work with me on getting a valid driver’s license right now, so I can’t drive myself anywhere. No support system, I have no family or friends left. I’m completely isolated. The control part with “Matt” is he insisted on paying for everything so I wouldn’t have to work and could be a stay-at-home parent for his 2 kids, 8F and 6M. He has full custody of the 6M because the bio dad is in prison for life. He pays my phone bill, car insurance, court fines, groceries, everything. We’re on a 1-year lease together. Landlord knows I don’t contribute income. Now he uses that against me. He threatens to shut off my phone or take it back. When I try to leave he says I can’t because “you’re on the lease.” It feels like I’m damned either way. Since we started dating he’s contacted my exes and old friends asking if I cheated. He recently found an old HS friend’s phone number and blew up all their phones telling them my life story + allegations. He works in Warrant Apprehension Unit and told me he “knows how to track people.” But is in denial of all of this. He somehow got my estranged dad’s phone number online when I never gave it to him. I’m terrified of what he could do if I try to leave.

*Health issues*

I’ve been on SSRIs for 8 years with severe permanent side effects, including loss of sensation down there, I have unidentified seizures + panic attacks in front of him and he doesn’t help. He just stares, or will use reverse psychology on me and tell me I am “the problem.” He never takes accountability for anything. Makes me feel like I am a problem because I am unable to have sex nor show any emotion in anyway because he’s stripped me of all my pride as well as the medication has really messed me up over the years, I’ve tried receiving therapy but they try to put me back on SSRI’s.

*Why I feel stuck*

No income, no license, can’t drive myself, no family or friends to stay with, on a lease I can’t afford alone, phone, car insurance, everything in his name, scared of his job and tracking access

What I’m asking, has anyone left a controlling relationship with zero income, no license, and no support system? What was step 1? Is this considered abuse even without physical violence? How do I safety plan when he has law enforcement/tracking access and knows my family info? Any resources for someone with seizures who can’t drive and has no one? I feel ashamed and exhausted. But I know I can’t live like this anymore. Any advice, resources, or just “you’re not crazy” would mean a lot. Thank you.
