Medical Work Form Without Insurance

I need to update and submit an annual medical work form. I currently don’t have insurance because of an unexpected gap period but I know it’ll be expensive out of pocket. Is there an affordable insurance here in NY I can quickly get just for the basics? Or what’s my best move? Thanks.
