Question about season 5 final 4 veto ceremony

SPOILERS FOR SEASON 5 BELOW

Why did cowboy’s decision to save Diane from the block immediately mean that nakomis was evicted? What if Diane wanted to vote cowboy out? Doesn’t make sense why there’d be a vote if the noms remained the same but wouldn’t bc he changed it?
