Law Firm and 3112c

If using an attorney, does/did everyone here write up a narrative (for 3112c) to give to your doctor, to edit and sign?

Or-

If provider agrees that condition will last a year, and you get restrictions as such, can that be forwarded to your attorney, and they will take it from there, as far as getting 3112c completed? TIA!

I'm trying to figure out best way to go for my upcoming doctor app; I don't want to overwhelm her with too much paperwork to sift through.
