Urgent Help Please: Residence Visa Medical Forms. Can They Be Notarized After Doctor Signs?

Hey all, I need some help from those who have gone through the process. TECO NY's website stated I could mail in my medical forms sealed by the medical facility or notarized. I just picked up the documents from my doctor's office yesterday. However, when I spoke to TECO directly, they said it HAS to be notarized only, plus a notarized copy.

THE QUESTION: How was the notary able to verify the health certificate without the doctor? Can I still get this signed or do I now need a mobile notary to go back to my doctor's office? Or are they just validating the authenticity of the document? I'm so sorry for the urgent post, but I was about to mail everything in today.
