Enraged by Gyn Follow-up on Deep Endo

Rant / Vent

Didn't know I had deep infilitrating endometriosis (let alone any endo). I went to the ER a few weeks ago for 3 weeks of bad pain and one night of severe unbearable pain (I get stuck kidney stones, so they should know that I know pain). Pain wasn't even controlled by morphine. CT scan just showed fibroids so they kicked me out 15 minutes after a morphine injection. Alone. Onto the streets of Manhattan.

Okay, fine. Followed up with a gyn. She really seemed to know what was up and ordered a pelvic ultrasound and later, to clarify results, an MRI.

Just had the most insane follow-up appt for that MRI ever. She clearly was just skimming the report for the first time on the video call, saying very incorrect things.

Results of the MRI are posted here.

What pissed me off beyond belief was:

- She was like "oh that symptom (pelvic floor or intestionl spasms) is unrelated" and didn't even ask me about other potentially related symptoms, like digestive symptoms.

- She read the MRI report wrong THREE SEPARATE TIMES. She said no fibroids, oh well, 1 fibroid. I have THREE FIBROIDS. Hydrosalpinx, which is not what the radiologist found most likely on the report. Endo with no reference to its severity.

- She told me surgery isn't necessary and she would prescribe a birth control. At the end of the call, she realized that **the birth control she picked interferes very badly with my mood stabilizer (**I have currently well-managed schizoaffective disorder - bipolar type and have my many meds combined perfectly right now). She then suggested that the birth control might "help fix my mood instead." (I was totally shocked and mentioned that I've gone through 10 months straight of life-ruining psychosis before, and she said that I was talking in circles). Prescribed that birth control anyway, so I guess she's a psychiatrist too!

- She was also like you have a little bit of fluid in your tube and I was like... can you just read this because I think it says it's likely that I have endo in the tube. So yeah, she's also a radiologist!

- She didn't have an answer when I asked what to do for my kidney stone ER visit level pain if it comes back (my main concern since I can feel it starting to come back). She just asked what the ER did, essentially said oh nothing?, and kept pushing birth control.

- When I said I didn't think I would have kids, she kept pushing for an IUD (I have Ehlers Danlos and know it would get embedded/scar... hell I have "extensive scarring" everywhere else, it seems).

- When I asked about all the scarring, specifically because it's affecting my rectum now, and everything I've read online about DIE, she said birth control was the answer and surgery causes will just cause more scar tissue.

I'm ripping out my hair right now. Asked for a referral to a specialist - giving me a name was the only thing she did right on the call. Am I going crazy or was I just horribly naive to think that this is 2026 and endometriosis is medically acknowledged now??

Full Report

IMPRESSION:

Scarring surrounding the right ovary and adnexa leading to a tubular structure with a fluid level that accounts for the findings at ultrasound. This may represent focal hydrosalpinx or a collection as a sequelae of deep infiltrating endometriosis or prior infection at the right ovary.

Multiple areas of scarring in the deep pelvis favors deep pelvic infiltrating endometriosis.

Multiple subserosal fibroids.

Findings:

Uterus size: 6.5 cm x 3.6 cm x 4.5 cm (series 3, image 19 and series 5, image 18).

Mild scarring at the posterior uterine body.

Fibroids: Multiple subserosal fibroids. ⁃ Posterior uterine fundus on the left 3.2 cm x 2.7 cm x 3.3 cm (series 3, image 1 5 and series 5, image 12)

Cervix: Normal morphology

Endometrium: 9 mm. Previously noted endometrial polyp is not evident

Adenomyosis: No adenomyosis Junctional zone is normal in thickness measuring 7 mm.

Ovaries:

-Right ovary: 2.7 cm x 2.8 cm (series 5 image 17). Contains physiologic follicles -Left ovary: 2.1 cm x 1.8 cm (series 5, image 17). Contains physiologic follicles and a small hemorrhagic cyst.

Adnexa/Other: There is scarring at the right adnexa leading to a tubular structure in the deep pelvis measuring 1.9 cm x 0.9 cm x 1.0 cm (series 4 image 26 and series 5, image 22) with layering hemorrhagic or proteinaceous debris. There is scarring between the right adnexa and rectum. No internal nternal enhancement at subtractior images.

Urinary bladder and Urethra: Normal

Lymph nodes: No enlarged lymph nodes

Bones: No aggressive lesion

Additional findings: No ascites

Extensive scarring in the deep pelvis
