Dreaded Banner of Death Question

Hi,

So I have a question to those getting the yellow/orange/red banners of death.

Do you get them every time you mention anything risky / explicit in your chats? Or is it like every other chat? I'm just wondering if it's always triggered by the codewords or if there are people perusing jailbreaks without issue for example for 2/3 chats of NSFW writing?

And a second question, the banners mention that user prompts violate usage policy. What if AI got trigger happy and generated something explicit? In theory, would that be exempt from that?

Just my middle-of-the-night musings.
