Confused about stenosis severity versus grading of anterolisthesis.

So my MRI shows a grade 1 anterolisthesis at L4/L5 but then it says the spinal stenosis is "severe". Doctor is pushing a steroid injection and due to cushings syndrome I try to avoid steroids. But my pain is often disabling and I am having some leg weakness but not all the time. Grade one doesn't sound so bad but severe stenosis sounds bad. I am a very poor surgical candidate due to other medical conditions.
