Parent in North Macedonia seeking a hospital that offers multidisciplinary (metabolic + neuro + GI) remote/telemedicine second opinions for a complex pediatric case
Hi everyone. I'm a parent in North Macedonia trying to find a top hospital that can review my son's full case remotely, and I'm hoping people here have direct experience with international remote second opinions.
The short version: my 4-year-old son is neurodivergent with no clear unifying diagnosis. His picture spans three areas that I think need to be looked at together rather than by separate specialists, and I'm specifically looking for a center that can do a true multidisciplinary team (MDT) review by telemedicine — not three disconnected opinions.
His profile (kept general):
Neurodevelopmental delay / autism-direction profile, being monitored, no formal diagnosis yet
Abnormal EEG findings, currently managed with valproate
Persistent gut Candida overgrowth (glabrata-dominant), being managed with diet + antifungals
Several mineral deficiencies (zinc, magnesium, selenium, iron)
Some metabolic/oxidative-stress markers on urine organic acid testing
Beta-thalassemia minor trait
He's been making real, encouraging progress lately, so this is about getting the direction validated/corrected, not a crisis
What I'm looking for:
A hospital with genuine depth in all three of pediatric metabolic/mitochondrial medicine, neurology, AND gastroenterology — not just one
A real remote MDT review where those specialists actually coordinate on one combined opinion
A pathway that accepts international patients from Europe / North Macedonia by telemedicine (we can't easily travel as a first step)
Ideally some sense of cost and turnaround, since we're paying out of pocket
Centers I've already researched and am considering: Great Ormond Street (London), CHOP (Philadelphia), Cleveland Clinic, Children's National (DC), Mayo Clinic Children's, and Cincinnati Children's. From what I can tell, GOSH and CHOP look strongest for an actual coordinated MDT, Cleveland is the cleanest confirmed international remote process, and Mayo's remote route seems harder to access from abroad — but I'd really value real experiences.
My questions to you:
Has anyone actually gone through an international remote second opinion at any of these (or elsewhere) for a complex multi-system child? How did it go, and was it genuinely multidisciplinary or just one specialist?
Are there centers I'm missing that are strong across metabolic + neuro + GI together and good with international families?
Any practical tips on getting an MDT rather than a single-specialty letter, or on packaging records for the best review?
Thank you so much — happy to clarify anything. Just trying to make the best possible decision for him.
