Public health specialist question

Hello slightly confused at the Public health system in England and public health consultant role. Why do LA’s post public health specialist roles but they aren’t consultant positions? Are they basically practitioner roles? Does this not undermine the public health specialist register? Considering the entry requirements are sometimes only a post-grad qualification…
