Asa maka request new Nat ID??

I lost my national ID last june 15 sa gaisano supermarket and i went there kanina to ask if may nakapulot or nakakita but wala daw. That was the only valid ID na meron ako huhuhu. Asa maka kuha ug new? And makuha ba dayun sya?? Huhu new here in Gensan rin po. Thank youuu!
