Birth registration Victoria

I’m a bit confused about how birth registration works in Victoria.
Will I be given a paper form at the hospital after birth, or is everything done online?
If we haven’t been in contact for months, what is the process for having him included on the birth certificate?
Does he have to complete his own section separately?
If I enter my daughter’s name first, can he change her first name or surname when completing his part?
I don’t intend to give my daughter his surname. I actually want to give her a neutral family name that belongs to neither of us. Has anyone done something similar, and did it cause any issues with registration?
If one parent enters the baby’s name first, does the other parent have to agree to it, or can they simply change it?
