What does it take for a teammate to get permabanned?

My teammate (gangplank) started running it down and ult+flashing my jungle camps since minute 15. How are players ALLOWED TO PLAY THE GAME after doing something like this

I was 12-3 doing fine we even got soul but literally any fight we lost because it’s 4v5 with one teammate doing nothing
