Is there anyone that should definitely know im doing hrt?

So I am going to diy hrt because its honestly the only feesable option for me atm and I want to know, who should definitely know and when? I know that I should tell my doctor, but should I tell him the next time I go or should I call to like pre warn about it or something? I'm just quite nervous about it ofc
