occupational health check

I’m starting GEM in August, and I only just received the occupational health check questionnaire from KCL.

I have had all the vaccines required, but they were done abroad, and I can’t get evidence of them. Do I just fill the form in and get the occupational health team to arrange for me to have a blood test/vaccines?
