Doc won't adjust my HRT

I started having a ton of Perimenopause symptoms after my hysterectomy in Jan, even though I kept both ovaries. My biggest symptoms were depression, anxiety, brain fog, joint pain, and urinary burning.

I was put on estradiol cream and the patch (0.375mg). The cream helped with the urinary burning a lot. However, my joint pain has only gotten worse. I'm in pain every single day. Sometimes I can't sleep at night because of the intense aching in my knees, hips, and ankles.

I called my provider to ask if my dose needed adjustment and she said no, it won't help with joint pain, then instructed me to go to urgent care and get labs done or start an antidepressant. The nurse actually had the nerve to say good luck before hanging up. Why would they not even entertain the idea of changing my dose?

Btw, I did get labs done yesterday (cbc, cmt, thyroid panel, iron panel) and all was normal.
