WOTC Form 8850

Hey all, I know the program am is in a confessional moratorium, but our state is saying we should still apply in the event it comes back and can go into effect retroactively. The problem is the DOL doesn’t have the pdf available anymore. We’ve searched all over only to find out outdated forms. Do any of you have a pdf saved? Thanks!
