Problem while manually entering data in Schedule 112A

I have just two trade details to enter in Schedule 112A. So I decided to enter it manually instead of using the CSV option. When I choose security bought "after Jan 31, 2018", the box for Name of share is auto filled as CONSOLIDATED, and the box for "ISIN" is auto filled as "INNOTREQUIRED". Can anyone suggest what option do I have to enter my trade details in Sch. 112A?
