Does this sound like anyone else's gout flare?
I’ve had recurrent “possible gout” flares for several years (probably 6–8 attacks). My uric acid has historically been elevated, usually around 9–10 mg/dL, but I’ve never had a joint aspiration or ultrasound to definitively confirm gout. My rheumatologist has never been fully convinced it was gout, while my podiatrist recently thought this flare was more likely a tendon issue because of my very high arched feet.
This flare started last Saturday with no traumatic injury. I actually woke up in the middle of the night with severe pain around the outside/back of my Achilles/peroneal tendon area. Over the next few days, the pain seemed to move. First the top of my foot became the most painful area, then by day 5 the pain was almost entirely on the inside of my ankle, with significant swelling, warmth, and I completely lost the ability to bear weight. I’ve been on crutches and in a walking boot for 2 days, yet the pain actually got worse instead of better. By today I ended up in the ER because the pain was an 8.5/10 and the swelling was dramatic. Morphine barely helped (8.5 → 7), but IV Toradol brought it down to about a 5.
The ER ruled out a blood clot, X-rays were negative, blood work was reassuring, and my uric acid during the flare was 7.2 mg/dL (even though it’s usually much higher). They couldn’t aspirate the joint because there wasn’t an obvious joint effusion. I have an MRI scheduled for Tuesday to look for tendon injuries.
My question: Has anyone else’s gout ever behaved like this, where the pain seemed to migrate from the outside of the ankle to the top of the foot and then to the inside of the ankle over several days with progressively worsening swelling? Or does this sound more like a tendon problem to those of you who have experienced both? I’m not looking for a diagnosis—just wondering if anyone has had a similar pattern.
