Got my pathology - awaiting plan, experiences?
Long story short im 36 year old man, had a normal scrotal ultrasound in Jan for fertility concerns, showing no signs of tumor. I started clomid therapy in Feb for fertility - LH and testosterone shot up nicely but developed a lump on L testicle end of May which showed "4 hypoechoic masses."
My doctors and me alike couldnt wrap head around this being a coincidcence, and even radiologist report stated "malignancy is not likely in context." Doctors said that both surveilling for 6-10 weeks or having orchiectomy was reasonable to see if it was a reactive hyperplasia not tumor.
Yet the imaging was very concerning so I decided to have L orchiectomy after freezing sperm.
Pathology came back today, aparently not 4 tumors but 1 tumor that is 3 cm. Non seminoma MGCT Embryonal predominant (85%) and teratoma (15%), with LVI and "abutting" the rete teste. and no spread to adjacent soft tissue or other structures.
CXR Clear no metastatic disease seen.
CT Abdomen pelvis clear no metastatic disease seen (reviewed by 2 centers).
Pre surgical tumor markers only significant for mild beta HCG of 5.5 - doctors also not sure if this was an artifact or tumor marker in my case as can be elevated w/ clomid therapy and very high LH, which I had from the clomid.
Wondering if anyone w/ similar pathology? From my reading it looks like they will recommend chemo but that doesn't address teratoma risk, but its only 15% of the tumor? Dopes that mean that they would likely recommend RPLND? Both? I doubt surveillance will be an option for me with LVI - despite there being no gross sign of metastatic disease.
Also aside: anyone ever have chemotherapy with a pregnant partner? We both really want to proceed with IVF using frozen sperm - even if I need treatment.
