Pain in hip/pelvic region, history of ovarian torsion
Age: 33
Sex: Female
Height: 5'5"
Weight: ~150 lbs
Race: Caucasian
Location: USA
Medical History:
- Right ovarian torsion resulting in removal of my right ovary
- PCOS
- Known 4 cm cyst on my remaining (left) ovary (last measured October 2024)
- Copper IUD (since 2019)
Medications:
- Cymbalta 30 mg
- Adderall 5 mg twice daily
- Spironolactone 100 mg
- Zepbound 5 mg weekly
Social History:
- Smoker
- Drinker (2-4 alcoholic beverages approx. 1-2 nights a week, but usually less)
- Heavy cannabis use (legal state)
Current Concern:
I'm worried I may be experiencing ovarian torsion on my remaining ovary and I'm trying to determine whether this is something that can wait until my scheduled ultrasounds on 7/3 or whether I should go to the ER.
Over the past 24 hours I've developed a fairly consistent dull aching pain on both sides of my hip flexor area(?) (between my hip and pelvis). Right side is more persistent and more painful. Along with that I've had:
- Dizziness on and off, but may be due to lack of sleep past few days
- Nausea (mostly improved now)
- Hot flashes with facial flushing
- Heavy sweating despite minimal activity
- Chills
- One episode where I became so dizzy that my vision blurred, I nearly vomited, and I had to immediately take a cold shower before I recovered. It was in the middle of my workday and was so sudden I was not able to alert my team before I stepped away (work from home)
Initially I assumed this was a hangover because I had 4–5 alcoholic ciders and stayed up late the past two nights (late Father's Day visit). While the nausea has mostly resolved, the dizziness and pelvic pain have continued.
I have experienced hot flashes since starting Zepbound, but they're usually triggered by walking several blocks outside in warm weather. And they don't typically come with this degree of dizziness or persistent pelvic pain.
My period ended 3+ days ago, so it is not menstrual cramps. I will note that it was unusual because I had much more severe cramping, and most of the bleeding was very dark/almost black.
In case it is related, or anyone can solve this mystery—I have had chronic pain on the right side where my ovary was removed since my surgery. That pain is different from what I'm experiencing now—it tends to be severe, sharp, stabbing pain that often occurs during my period, lasts 60–90 seconds at a time, and makes me instantly stop what I'm doing it hurts so much. Best described as feeling like "someone jamming a rusty spoon into my hip." 8/10 pain. Can happen at random times throughout a day or back to back with brief periods of relief between. Worse with activity. Multiple scans haven't found a cause. An ER physician once suggested a possible hip bone spur, but my PCP later ruled that out. My gynecologist has wondered whether scar tissue or connective tissue could be contributing, but nothing has been confirmed.
Just FYI—I used ChatGPT to help me organize my symptoms, as I am in pain and having a difficult time lining them up in a way that makes sense. I typed this last bit entirely myself to give an example of my general mental state/ability to string a coherent sentence together.
