Is this an interoception issue?

Help me, I beg of you 😭

We've been at it for months and I'm at a breaking point! My almost 3 year old WILL NOT (can not??) initiate going pee in the potty. She will pee on demand if we remind her, but she has never told us she has to go potty. We've caught her starting to have accidents and she can hold it while we make a mad dash to the toilet, but if we don't see it she will just have accidents.

I've tried everything I know of to help with this (modeling, talking about all the sensations our bodies are giving us, books, videos, literally pushing on her bladder to demonstrate that it feels different, etc.) We've done different kinds of rewards, a potty watch, and picking out special undies.

She's a rather independent kid and it doesn't seem like laziness. She is also genuinely upset and feels a lot of shame when she has accidents. She showed all the readiness cues and needs to be potty trained by the fall for school.

Additionally, she's got some mad constipation issues (she had it prior to potty training and it's a different battle we continue to fight) and I know that can sometimes mess with sensations and interoception.

PLEASE throw your suggestions at me. I will try anything.
