GP refused to prescribe as per Psychiatrist recommendation

Questions/Advice

27M in Australia.

I’ve been taking Vyvanse 30mg for close to four weeks now. Medication is overall working really well for me. Minimal side effects. Improved mood, executive functioning and focus, and sleep. I take it at about 7am, and most days the effects last until 5pm which is perfect. However, some days the effects wear off at around 3pm.

I went to my GP to discuss this and requested a 1-3mg booster of dexamfetamine booster on the off days, likely once or twice per week, I experience the crash and need to remain focussed for work.

My GP completely rejected the idea. He kept saying ‘I am not comfortable prescribing it to you. If the psychiatrist wants you to have it, they can prescribe it’. Instead, he was happy to increase my Vyvanse to 40mg per day, despite it being effective majority of days in the week. I tried to advocate for myself, saying I don’t really want to increase my dose yet and that I would really prefer a small booster when required. He outright refused saying he will not consider this. He began to get heated and expressed how ‘pissed off’ he was that psychiatrist just handball prescribing responsibilities.

He pulled up the psychiatrist letter in front of me and refused to acknowledge the part where the psychiatrist stated ‘if the patient experiences a crash in the afternoon, 1-3mg of dexamfetmaine is appropriate’.

Overall a very disempowering experience. I am a psychiatric nurse, so I was appalled by how I was treated. My GP has otherwise been a great support through previous struggles so this was a really tough experience.

Has anyone else experienced something like this? How did you navigate it?
