Sudden short-term memory loss and confusion after manic episode 33F

33F had a psychotic manic episode about 3 months ago lasting approximately 3-4 weeks, relapsed on meth and fentanyl for 3-4 days after a month sober after episodic use for a year. Has since experienced short-term memory challenges after manic episode, will occasionally get up from seat at home and work, roam around but forgets what they’re doing and then sits back down and has had two episodes of extreme confusion, not knowing who they were, where they were and was unable to read a map. One of these episodes lasted about a minute and the other was upon awakening and lasted about five. There were no vision changes, sudden physical weakness or any other symptoms. No current mania symptoms but does have residual psychotic symptoms including auditory hallucinations and mild delusions. No command hallucinations, never a voice that can even be understood. No current drug use for over 2 months, occasional drinker of 1-2 drinks, so far episodes happen when sober.

Blood work so far has included CBC and B12, all normal so far except B12 is at 170 pg/mL.

Family history includes a maternal grandparent with early onset dementia at age 65 and Parkinson’s, and a maternal cousin with Parkinson’s diagnosed in her 20’s.

Medications include Risperidal consta 25mg biweekly, benztropine 1mg prn (about 1x weekly), adderall xr 40mg (about 3-4x weekly).

Any ideas?
