This is weird

So I never get outbreaks hardly ever as far as I am aware on my vagina BUT I do feel tingling itching constantly in my butthole (not to be graphic but yes) and sometimes it feels as if there’s sores in there and other times no but if there is you can’t see them. I often get hemorrhoids and fissures as before I did not… everytime I go number 2 it hurts to some extent and has never been the same . Is it possible I have permanent outbreaks down there and what can I do to end this? Should I take daily suppressive therapy? Because as before I only took meds during an outbreak but I feel like maybe I need to do lifelong meds to stop this
