22F — pulsating in my right ear matching my heartbeat with a right-sided headache. Should I go to the ER?

22F, 5’11”, approximately 185 lbs. I take Zepbound 2.5 mg once a week and use tretinoin 0.05% cream. I used to smoke but no longer do. I have juvenile rheumatoid arthritis.

About three hours ago, I started feeling and hearing a pulsating sensation in my right ear. It comes and goes, but it is there most of the time. It feels like a heartbeat or like there is water in my ear, and it matches my actual heartbeat.

Now I have also developed a headache on the right side of my head. This is the first time I have ever experienced anything like this. Should I urgently go to the hospital, or is it okay to wait and see if it stops?
