Still confused about how my provider submits authorization/forms for the GLP-1 bridge program?

Hi all? I’ve been doing a lot of research online about the GLP1 program that starts in two days on July 1, 2026.
What I’m unsure of is how my provider submits me for this.
I’ve seen things that a form needs to be filled out, which is viewable online, but it has the fax number blurred out.
But I’ve also seen that a provider can just submit the medicine and it’ll go to a prior authorization automatically to the pharmacy and then get approved.
Does anyone have any concrete information on this?
