Unusual bleeding

I am a 61 yr old female on blood thinners complete Hysterectomy in 2021 have blood coming from vagina anytime I bare down like to release bowels or pick up anything heavy or am stressing hard .GYN ran test nothing unusual about vagina etc..doctors said must be BLOOD THINNER when my ear bled for no good reason ER said same thing. I feel like something isnt right with the amount of blood coming from vagina its like a period
