Looking for 3D print commission

I’m not too familiar with how to order a custom 3D printed piece so I’m looking for suggestions for businesses that would do this.

Looking to make a square box, roughly 5x5 inches, with an icon of my business’s QR code in the center and my business name below.

If anyone has any recommendations for top websites to order from that would create this for me. Preferably a Canadian based company
