Interview anxiety

I (22 M) had my first interview 3 months ago, and it went terribly to the point where I stuttered my own name and forgot it, even though I had prepared myself. Ever since, whenever I try to apply for a job, I start to shake, thinking it will be just as troublesome. I genuinely don't know what to do.

Any advice?... please
