Made a skill that stops Claude Code from claiming "done" when it isn't

Claude Code Workflow

Agents keep telling me a task is done when it isn't, and sometimes they edit a test so it passes and then report success. So I made make-no-mistakes.

It freezes what "done" means before any code is written, catches the agent if it tampers with a test (the run halts instead of pretending), and makes a separate verifier that never wrote the code re-run every check before it's allowed to say done. If it can't verify, it tells you it's stuck instead of faking a green.

It does not promise zero bugs, nothing can. It just makes sure no unverified work gets past you.

Open source, MIT, no telemetry. Works with Claude Code, Codex, OpenCode, or any Agent Skills runtime. If you install it and manage to get a fake "done" past it, that's the issue I most want.
