Is it reasonable to ask for a Focalin dose increase after only a week? Feeling frustrated and running out of options
I’m a 32-year-old mom of four and I’ve struggled with what I now recognize as ADHD symptoms for as long as I can remember. I’m generally intelligent and capable in many areas, but I have this bizarre disconnect where I can be extremely organized and structured with some things, yet completely unable to make myself do other simple tasks.
Examples:
-Sending an email I don’t want to send
-Literally knowing what I need to do and just not doing it because I don’t feel like it.
-Doing dishes or basic household tasks when I’m overwhelmed
-Completing work tasks that feel mentally “blocked”
Over the last few years I’ve tried several stimulant medications, including Ritalin, Vyvanse, and Adderall. Adderall initially helped, but eventually I started feeling like I was chasing effectiveness rather than actually functioning better. Around the same time, I lost my job, and the stress of unemployment, finances, parenting four daughters, and trying to stay healthy has honestly made me feel like my life is falling apart.
My doctor recently switched me to Focalin IR 5 mg twice a day because Adderall wasn’t working well anymore. I’ve been on it for about a week. The good news is that I actually like it better than Adderall in some ways—it feels less intense and less artificially motivating, which makes it seem more sustainable long-term.
The problem is that 5 mg twice a day does not feel like enough.
My concern is that my doctor has mentioned she’s worried about getting “flagged” because we’ve changed medications several times recently. I’m worried she’s getting frustrated with me, even though I’m not trying to medication-shop, I’m just trying to find something that actually helps me function.
I also tried Wellbutrin, and while it reduced my desire to take stimulants, it made me feel emotionally flat and oddly angry, so that wasn’t a good fit either.
So my questions are:
-Is it reasonable to ask for a Focalin dose increase after only one week?
-How long do doctors usually want you to stay on a starting dose before adjusting it?
-Has anyone else felt that Focalin was helpful but needed a higher dose to really notice the benefits?
For context: I’m 32, about 220 lbs, physically active (10k+ steps/day), eat fairly healthy, and generally sleep well. I’m not looking for a euphoric feeling—I just want to be able to consistently do the things I need to do without feeling mentally paralyzed.
Would appreciate any experiences or advice.
