What is my doctor going to say?

I have my in person doctor appointment in 2 weeks, I have Kaiser insurance and Ive recently learned they do cover removal! Im wonder what stage it needs to be for it to be covered? I have a lot of pain, will that be enough for the surgery to be covered? Any advice would help!
