Cage slipped off?

I recently got a few cages one was two large and another did not sit right I got a perfect sized tube but it pushes down and a decent amount of shaft is left exposed when erect I got a smaller ring size than before to but for the first time it just popped off?

I might post a picture of how it looks on my body at some point but I’m not sure what went wrong
