Missing C&P appointments in decision letter

Good afternoon! I was wondering if this had happened to anybody else. My claim was deferred and they scheduled me for 3 C&P appointments. My last C&P appointments was July 5th and 6th.

I just got my decision letter today and the VA only addressed C&P appointments to June 30th and did not mention any of the claims that the 5th and 6th C&P appointments were for in the letter.

I called the VA to get clarification and they submitted a request saying that they see two of the contentions have not been addressed , has this happened to anybody else?
