New role or old role (same salary/benefits)?

Jobs, Saturation, and Salary

I am in a little bit of a dilemma. I have been with a large PBM company for 3 years now as a contractor pharmacist. I have been asking to be moved to permanent full time as there were NO benefits (not even PTO or sick days) with being a contractor, but was told about budget constraints. I started applying to jobs, and luckily got a Utilization Management Pharmacist role with a much smaller PBM company. Just days after accepting the UM offer, my manager said they had decided to offer me a permanent role. The thing is while I am tired of my first job because it has gotten monotonous, now that I am a full time employee there is a better chance of me moving within the company to a different department where I can do something different eventually (possibly still UM). The new role is a much smaller company, with a VERY small team and I do not have experience in UM so I have a lot to learn but the day to day tasks vary which I like. The pay and benefits is exactly the same, one has slightly PTO than the other by 2 days. While I am excited to challenge myself in this new role by learning new stuff, I have stuff that just came up in my personal life that has caused me to be all over the place in terms of living. But that will be sorted soon. So the first job is with a big company (with opportunity to move to a different role now I am permanent), I'm basically an expert at what I do (though I am tired of it) but I really believe I am just a number there. The second job is with a much smaller company and small team, new role with an opportunity to challenge myself now though I don't really see much opportunities for career advancement within the company. If anyone hs been in similar situation, how did you make a decision, what decision did you make, and did you have any regrets. Also, what factors should I consider most.
