How does honorable combat work, and in what scenario is it disabled?

It seems like some zones don't have honorable combat enabled. From what I can tell, STV and alterac mountains doesn't have it.

But are there other cases where it would be disabled? Like mercenary or high risk?

I'm a bit confused on the nuances of honorable combat.
