How does the deck work

So, with Stateside Scramble, ive been wondering how the decks work?

I.e.

Team A pulls Florida. Does Team B then have to search their deck for Florida to remove it?

If they do, then surely they can work out what hidden cards there are by seeing which states aren't in the deck?

Or if they pull a card already pulled do they just reshoot?

Also, can't remember it being mentioned, but seems like removed cards are fully removed? They dont go back into circulation?
