Where can I go to have a doctor fill out paperwork for cheap? No tests needed.

I'm applying for a job where they asked me to have a doctor fill out some paperwork. It's more about past events, such as fainting, and no new tests need to be done. I just got a new GP and their office charges $75 per form.

Do I have any alternatives? Do some doctor's offices charge less than others for paperwork?
