Thoughts on TMJ patient case
Hey, I wanted to see if the collective hivemind had any helpful advice. I work in outpatient, I occasionally see TMJ patients, and I'm literally the only person in my entire region that's willing to even try with TMJ (it's not my favorite but I do what I can). Recently I got someone referred to me, and their chief complaint was R sided deep ear pain that is worse *when they put their hearing aides in*, and is better when *they take their hearing aides out*. They don't have pain eating or chewing. No issues with opening. Nothing else in daily life makes it worse. Just the hearing aides, but it's also apparently completely random and some days are worse for no discernable reason. Like some days it hurts a lot and they haven't even work their hearing aides. I don't know. But they've had some traditional TMJ issues on that side before, they're kinda hypermobile overall, and they have some R sided clicking with opening. They also have reportedly weirdly shaped ears and they have a hard time finding hearing aides that fit and work well in their ears. And their ENT says there's nothing wrong with their ears, and they should try some TMJ therapy to see if that does anything. My first thought was this has nothing to do with their TMJ, but I can try a few sessions working on R sided muscles and maybe some distraction and dry needling and see if it's just a trigger point thing that's referring to the ear. Here's the big thing we're maybe realizing. The pain is better in supine. She never realized this on her own. But when she happens to be flared up, hearing aides or no hearing aides, supine helps. I've tried some soft tissue and distraction and it's hard to tell if that's helping (I'm gonna dry needled a little bit next). But we discovered being in supine helps more than anything I've tried so far. What the hell could be causing deep ear pain that's worse with hearing aides (sometimes) and gets better in supine? I already told her to message her ENT and let them know this. This feels medical and not MSK to me. Thoughts? Anything?
