Nurtec helps, but it doesn’t fully stop my migraines - what else should I ask my neurologist about?

I’ve had migraines my entire life, but they used to happen only about once a year. When I turned 27, I suddenly had a huge cluster of them, and ever since then I’ve gotten anywhere from one to four migraines a month. (I’m 33 now)

I know that isn’t considered chronic compared with what many people here experience, but they’re extremely disruptive when they happen. I usually get an aura first, followed by pain, numbness in my hands and sometimes my tongue, and then vomiting that can last for around six hours.

Nurtec has been my saving grace. When I take it early enough, it usually stops the vomiting and severe nausea, which has made the attacks much more bearable. However, I still have head pain, feel strange and out of it, and usually need to lie down for five or six hours. I’m still missing plans and having to leave things early whenever one hits.

Sometimes I’ll get another round of aura during the same migraine. I’ve occasionally taken a second Nurtec when that happens and felt like it helped (my neurologist said I could)

I’m wondering whether I should try a different abortive medication or whether there’s something people commonly combine with Nurtec that can stop the attack more completely. I read posts from people who take medication, feel better within an hour or two, and are able to continue their day. I would love to get closer to that.

Advil, Aleve, and other over-the-counter pain medications don’t seem to touch the pain for me. I have an appointment with my neurologist next month and plan to discuss preventive medication as well, but I’d love to hear what medications or combinations have worked for people with similar symptoms.

What would you suggest asking my neurologist about? Or anything to add on my own? I’m in a weird middle place of not having enough migraines to warrant trying like a preventative injection but enough that they’re really fucking me up.
