So can you actually fail the special orders?

I’ve played this game twice and I’ve always wondered if it was possible to fail the special orders like knocking off Kamoshida’s treasure or missing the harpoon when going against the shadow Wakaba.

Has anyone ever missed or failed it or will it always work.
