TRC application details through MOS

I submitted my application for a temporary residence and work permit through MOS on 3 July 2026 to the Podlaskie Voivodeship.

I noticed a few details in my submitted application that I may need to clarify or correct:

For place of residence/stay, I entered Białystok address because I will be relocating there for my new job. I asked my prospective employer beforehand if I could use the accommodation address they will provide (same address of the school/employer) and they confirmed that I could. However, at the time I submitted the application, I had not physically moved there yet, I will move there in August.

My marital status shows “kawaler” instead of “panna.” I am female and single, so I believe this is an incorrect selection/wording.

The date of my last entry into Poland appears blank.

The section regarding my previous stays in Poland also appears blank.

My application currently still shows “SUBMITTED” in MOS and I have received my UPO.

Has anyone had similar mistakes or missing information in a MOS application? Did the Voivodeship Office first send you a request for supplementation/correction (wezwanie do uzupełnienia braków), or can these types of mistakes lead directly to refusal?

Also, for applications submitted through the new MOS system, did you receive requests for additional documents through MOS/email or by physical post?
