Any ToA experts? Weird death skull skipping

Done hundreds of 400+ 500s solo aswell, always been skull skipping and never died. Meanwhile when going duo and skull skipping sometimes randomly i get 1 shot when doing exactly everything the same. But not always. Just randomly. Sometimes i die, sometimes i don't. Whats the reason for the death?
