Vaccination

What do they mean by "At least proof of one of the above vaccines is required for making a Medical folder"

Are we supposed to make a medical folder during the first week and how common is it for people to not have vaccination documents with them at the time of registration. I have my vaccination records but it doesnt have my name on it so am i supposed to take the vaccine again?
