Severe abdominal cramps, nausea, bloating following gallbladder surgery and ileus

Hi it’s me again, with my unrelenting tummy issues that I’ve literally never had before three weeks ago. Posting here because I can’t get into my doctor today at all and I really am trying to stay out of the ER because I have a criminally high ($2k) copay and I’d like to not add to the life debt I owe the hospital.

Context: 4 weeks ago I woke up in the middle of the night with excruciating abdominal pain and nausea, went to the ER, imaging showed it was gallstones and gallstone pancreatitis and I had emergency surgery but had to stay a few extra days due to the pancreatitis.

The following week my pain increased (all in my upper stomach), I had nausea and I went four days without a bowel movement so my doc sent me back to the ER, where I was admitted for an ileus. Hospital pumped me with laxatives and an enema and sent me home once I was having BM consistently and not puking with instructions to take MiraLAX and senna twice a day then taper down to once a day and eventually stop.

When I stopped it, I stopped having BM and went 4 days without one, saw my pcp again and she recommended to start it again once a day. I started again on Friday, had a teeny BM Saturday, two almost normal size BMs yesterday.

Then at 3am last night woke up with really bad cramping pain and LOUD gurgling sounds in my tummy and a massive pressure and feeling of fullness. This time the pain is more spread out but is really mostly in my lower abdomen. I cannot have a BM, not passing since 6am despite feeling like I need to, nauseous, pain is worse when I’m sitting or hunched over. Super bloated but I’ve been bloated for the last month pretty much so that’s not new.

The only really labs from my dr last week (done Friday) was still an elevated lipase (382 with the high end of normal being 56), ketones in urine, everything else was pretty much normal.

I’m not dumb so I’ll obviously go in if I need to, but I figured I’d get a professional opinion before wasting $2k and being told this is normal and is just my post-ileus bowel starting to kick into gear lol.

Thanks!!
