Anthem Blue Cross of CA Billing as Chiropractor

Hey guys - Are you coming across Anthem Blue Cross of CA claims by commerical plans processing E/M 99203 and 97110 and 97140 as not included in the Chiropractic copayment for spinal manipulation? We are receiving claims back for a $15 copayment but it comes back as the patient responsibility of allowed amounts being passed to the patient.
