Help regarding Anti Ragging Affidavit

Can anyone please explain the steps in the process of making the Anti Ragging Affidavit for admission?

Do you print out the undertaking on regular A4 page or on a non judicial stamp paper of 10s?

Do you have to submit it on the website of antiragging afterwards? Or register on the website beforehand?
