Really struggling

I was on zepbound for a year a year ago. I lost about 60 pounds. I was on the next highest one Whatever it is dose. It made me have colitis. I was very very sick. Had to get off it. Gained so much weight back. I have adrenal insufficiency and many other contributing medications that make weight loss very very difficult. How can I improve stuff? I’m not eating whole bunch either. I’m on my third shot for the five and have seen zero progress.
